Der Heilige Militärkonstantinische Orden vom Heiligen Georg wird vom Großmeister geleitet, der von der Königlichen Deputation unterstützt wird, die aus den Großchargen besteht: Großpräfekt, Großprior, Großkanzler und Großschatzmeister, Präsident, Vizepräsident, Sekretär und Deputierte.
Der derzeitige Großmeister, Seine Königliche Hoheit Prinz Karl von Bourbon und Sizilien, Herzog von Castro und Oberhaupt des Königshauses, wird außerdem von einem juristischen Magistralrat und einer heraldischen Magistralkommission unterstützt.
Der Großmeister ist der ständige Verwalter des Ordens, kann neue Gesetze und Statuten erlassen sowie die höchsten institutionellen Chargen ernennen, denen die moralische, disziplinarische und administrative Leitung des Ordens anvertraut ist.
Der Großmeister wird bei seinen Entscheidungen von einem „Berater für das Königshaus und die dynastischen Orden“ unterstützt, der vom Großmeister selbst ernannt wird und eine Person von höchstem Vertrauen ist.
Der Großmeister kann einen „Generalsekretär“ ernennen, dem er die notwendigen Befugnisse zur Leitung und Koordinierung der Tätigkeit der Kanzlei und der Delegationen überträgt.
